Volunteer Task Completion Form

Volunteers can submit items via this form to complete a task from the list above and/or evidence the completion of a task from the list above. Loyalty Points will be awarded automatically upon submission of the form, but can and will be revoked if evidence proves insufficient in fulfilling the criteria above.

Please note that some tasks are completed/submitted using different forms (WNTPRM tasks, external jobs, etc.) and cannot be submitted through this form.
